State license numbers may be obtained by a name search.
To download this file directly to you PC. right mouse click here and select the option to "Save Target As...". You may also open this file directly into Excel™ from this site by clicking here.
The file is in CSV format and can be easily opened into any database such as Excel™, Access™, etc. This is a very large file with almost 30,000 lines of data; therefore it make take a while to download or open. If you do not have a database application you can download an Excel™ viewer.
When opening the file into Excel™ and Access™ you will need to resize the column widths. The table below lists the data array by columns:
column 1 = state license #
column 2 = Medicaid Prescriber ID
column 3 = Last name, First name, Middle initial, Suffix
column 4 = street address
column 5 = city
column 6 = state
column 7 = zip code
